A) The shows change from WOW to some kind of damned soap opera format.
B) "Killing" Juliette and then bringing her back as "Eve" was just downright stupid!
C) Not knowing what to do with the character of Truble. Bringing her in for a few episodes and than making her disappear for half a season was idiotic!
D)Constantly dropping plot lines without resolution. All of us here have discussed this subject. Plot lines were introduced to the show and then just disappear without explanation.
And finally, I think several key cast members and the show runners were tired of doing the show. DG, JK and David Giuntoli have been making noises for the last year or more that the show had about run its course as far as they were concerned.
For example, back between S4 and S5, Giuntoli hinted in a couple of interviews at Comic-Con that he thought the show might end soon.
This thread title, 'What "killed" Grimm?' reminds me of "Who Killed JR?"
Your point B ("Killing" Juliette and then bringing her back as "Eve") is IMO the primary reason Grimm died. I recall when the writers assured us Hexenette was dead and how many reacted:

The title line says it all:
