In Juliette's case it would probably be the guilt of having helped facilitate Kelly's death combined with some sort of unspecified drug and psychological conditioning by HW. I don'r think there are any cases on record of a "split personality" being created through deliberate effort, so we would be talking science fiction here.
But then we go back to the issue of whether Juliette was/was not telling the truth when she told Nick she didn't know Kenneth was going to kill Kelly. In any case, I don't really think that was a factor. While the series tried to bring some major shock value into the "head in the box", it really wasn't all that shocking.
And Juliette knew that Kelly was no innocent by virtue of being a Grimm, and from what we know of the Grimms, they seem to die violent deaths. She would have known that. She also would know that Kelly would have taken Diana regardless. It was just a case of a bunch of criminals kidnapping a baby who was then kidnapped by another bunch of criminals. I still think Juliette was getting revenge on Adalind. Kelly was just an obstacle that got in the way.
I would be thinking that, if this was some kind of split personality disorder, Juliette would be suffering periods of black out.
