I think we saw in "The Naked Time" that Kirk has a dedication to maintaining "command presence" that borders on obsession. Or possibly crosses the line into it. The few times we see him relax it with Spock and McCoy are always in private. And the younger crew members he runs into whose parents were people he considered mentors in Starfleet is a tell that this is probably not something that was taught to him at Starfleet Academy. He's emulated the on-the-job behavior of people he's admired in the past, but never learned how to turn it off for an actual personal life. His lack of a personal life and personal relationships outside of Starfleet eventually becomes the overarching theme of the theatrical films.
I think the most likely candidate for "the little blonde lab technician" during the series was Janet Wallace, the doctor in "The Deadly Years." But Carol Marcus also became a leading candidate when ST II came out. No clue where he might have known Ruth from; the dress and rose make me think "high school prom date," but the actress was too old for that.
