That's such a good question. Diana has had a thing with skulls even when she was a baby. Plus, she did create the skull on the window just before the King got thrown out of the helicopter. She showed/made a skull on Juliet's mirror when Juliet intercepted her psychic message to Adalind.
Now skull guy showing himself to Juliet via mirrors.
Maybe that's the connection- Diana and skulls- skull guy coming, the prophecy?

The Skull thing is a Hexenbiest thing. The Skull showed up in Adalinds Suppressant potion to let people know it is done and on the Paste to restore Adalinds powers.
So I believe it relates some way to Hexenbiest powers. Not just a Diana thing but a general Hexenbiest thing.

I always felt the skull that appeared when Adalind rubbed the paste on her pregnant belly was indicative of Diana's future self. The skull appearing in connection with the suppressant spell would be concerning Adalind alone. Even if we accept the premise that it's a Hexenbiest thing, we must then agree that only Diana has used it extensively, so much so, that I always equate it with her alone. Still, it works against Diana being exclusively associated with the skull image.

It still provides a link between Diana, her past, and the skull monster (who is supposed to be Nick by some accounts). Guess we'll know a lot more after this next episode that may have something to do with blood and conjuring up the skull guy.
