Kelly appeared to have arrived in Portland because she was trailing the four Schakals who had been responsible for the Rhinebeck attack. So yes, it apparently did take 20 years. It didn't appear that she had any intention of contacting Nick until she followed Kimura to Nick's house and saw that Nick was being attacked. So she was still "protecting" Nick by not leading her enemies to him, until they found him on their own and she had to jump into the fight and reveal herself.
Nick clearly disagreed with Kelly on all of this, and Kelly eventually told Nick that she regretted her decision. However, at the end of the multi-part episode, Kimura and his buddies were all dead but she still didn't seem to have altered her life at all. Maybe she still had a boatload of other enemies she'd picked up over the years. Or maybe after 20 years on the run/hunt she was just unable to change, settle down and become a mother again.
It seems a simpler solution would be to put Nick up for adoption.
It's not as simple as that because Kelly knew he might become a Grimm someday. She didn't know he would be a grown man when he awakened his sight so leaving him with someone who could relate to him was probably the best option or he'd end up with people who thought he was crazy and couldn't help him in any way.
